Dari duka ke delusi: manifestasi gejala psikotik pada pria yang mengalami significant loss

Abstract
Pengalaman kehilangan signifikan merupakan peristiwa kehidupan yang menimbulkan tekanan psikologis mendalam, terutama bagi individu rentan. Penelitian kualitatif studi kasus tunggal ini mengeksplorasi dinamika transisi duka ke delusi serta manifestasi gejala perseptual dan kognitif pada pria berusia 29 tahun yang kehilangan figur ayah dan tante. Data dikumpulkan melalui wawancara klinis (autoanamnesis dan alloanamnesis), observasi, studi dokumentasi, serta analisis riwayat perkembangan. Keabsahan temuan dijamin melalui triangulasi sumber dan teknik. Melalui integrasi data secara tematik untuk mengidentifikasi keterkaitan stresor masa lalu dengan kondisi klinis terbaru. Digambarkan dari hasil penelitian bahwa subjek menunjukkan kesedihan yang dalam, kehilangan makna hidup, dan menolak kenyataan. Terdapat berbagai gejala psikotik seperti halusinasi auditorik dan waham paranoid yang muncul di saat bersamaan dengan adanya gangguan afektif, yang menggambarkan adanya peningkatan risiko gangguan persepsi realitas sebagai manifestasi proses berduka yang kompleks. Terdapat distorsi secara kognitif berupa overgeneralization dan catastrophizing serta kecenderungan membatasi diri. Dari temuan ini ditunjukkan indikasi kuat bahwa respons duka kompleks berada pada ambang batas gangguan serius. Integrasi data mengungkap bahwa distorsi kognitif dan pengalaman perseptual merupakan manifestasi duka tidak adaptif, yang diperburuk akumulasi stresor lingkungan seperti beban pengasuhan dan trauma perundungan masa lalu. Oleh karena itu, deteksi preventif, solidaritas eksternal, dan mitigasi mental menjadi fondasi utama dalam memediasi transisi adaptif individu.
