Fetal Hydrothorax pada Kehamilan Preterm: Laporan Kasus

Abstract


Latar Belakang: Fetal hydrothorax adalah kondisi langka berupa akumulasi cairan di rongga pleura janin yang dapat menyebabkan gangguan kardio-pulmoner dan komplikasi serius. Prognosis bervariasi, sehingga deteksi dini melalui ultrasonografi prenatal penting untuk menentukan penatalaksanaan dan luaran perinatal.

Kasus: Seorang wanita 36 tahun, G4P3A0, usia kehamilan 23 minggu, datang dengan keluhan sesak napas. Ultrasonografi menunjukkan efusi pleura kanan tanpa hydrops fetalis dengan biometrik janin sesuai usia kehamilan. Diagnosis fetal hydrothorax dextra terisolasi ditegakkan dan dilakukan penatalaksanaan konservatif berupa pemantauan ultrasonografi serial, terapi suportif termasuk methisoprinol, serta rujukan fetomaternal. Evaluasi usia kehamilan 28 minggu menunjukkan efusi pleura bilateral dengan perbaikan kualitatif dan tanpa tanda progresivitas.

Kesimpulan: Fetal hydrothorax dextra terisolasi usia kehamilan 23 minggu tanpa hydrops fetalis dapat dikelola secara konservatif dengan pemantauan ultrasonografi serial, dimana diagnosis prenatal dan evaluasi berkala berperan penting dalam menentukan tatalaksana, prognosis, dan luaran perinatal.
