Monitoring Analysis of Filling The Informed Consent of Blood Transfusion

Abstract


Approval of the actions of medicine (informed consent) is the consent given by the patient or next of kin after getting a full description of the action of medicine or dentistry will be done with the patient in the form of medical treatment in the form of preventive, diagnostic, therapeutic or rehabilitative committed against a patient. Actions based on a certain level of probability that can result in disability or death. In the space Baitussalam 2 Hospital Sultan Agung Semarang p roses giving informed consent been going well, but there are obstacles in the documentation, namely often forgetting, this is because the number of nurses is not enough with a lot of action. This can be seen from the records of the patient's status with the action of giving blood transfusions where there is no agreement sheet for giving blood transfusions as much as 55%. From this, students concluded that the patient's completeness of informed consent was incomplete, this could reduce the quality of the hospital. The aim of this study is determining of monitoring of the filling of informed consent documentation for blood transfusion. This study used the interviews, observation and document study by developing an approach to 8 nursing management functions, organizing functions, personnel, direction, supervision, care management, logistics management, quality assurance programs and patient safety. After observed for one week, starting from 24 November 2020 until December 1, 2020, to get the data that: within one month there were 20 patients with the action of blood transfusion, 9 record patient medical records are written informed consent to complete, whereas 11 patient medical records did not have informed consent. The problems that arise are a non-accessory medical record of patients, especially in the provision of blood transfusion, not optimal supervision in the completeness of the medical record patient especially informed consent administer blood transfusions. After re-education to the head of the room, team leader and staff of informed consent documentation, giving blood transfusions increased to 80%.
